Heart failure biomarker levels correlate with invasive haemodynamics in pulmonary valve replacement

Background:
Although widely used in cardiology, relation of heart failure biomarkers to cardiac haemodynamics in patients with CHD (and in particular with pulmonary insufficiency undergoing pulmonary valve replacement) remains unclear. We hypothesised that the cardiac function biomarkers N-terminal pro-brain natriuretic peptide (NT-proBNP), soluble suppressor of tumorigenicity 2, and galectin-3 would have significant associations to right ventricular haemodynamic derangements.
Methods:Consecutive patients ( n = 16) undergoing cardiac catheterisation for transcatheter pulmonary valve replacement were studied. NT-proBNP, soluble suppressor of tumorigenicity 2, and galectin-3 levels were measured using a multiplex enzyme-linked immunosorbent assay from a pre-intervention blood sample obtained after sheath placement. Spearman correlation was used to identify significant correlations (p ≤ 0.05) of biomarkers with baseline cardiac haemodynamics. Cardiac MRI data (indexed right ventricular and left ventricular end-diastolic volumes and ejection fraction) prior to device placement were also compared to biomarker levels.
Results:NT-proBNP and soluble suppressor of tumorigenicity 2 were significantly correlated (p < 0.01) with baseline mean right atrial pressure and right ventricular end-diastolic pressure. Only NT-proBNP was significantly correlated with age. Galectin-3 did not have significant associations in this cohort. Cardiac MRI measures of right ventricular function and volume were not correlated to biomarker levels or right heart haemodynamics.
Conclusions:NT-proBNP and soluble suppressor of tumorigenicity 2, biomarkers of myocardial strain, significantly correlated to invasive pressure haemodynamics in transcatheter pulmonary valve replacement patients. Serial determination of soluble suppressor of tumorigenicity 2, as it was not associated with age, may be superior to serial measurement of NT-proBNP as an indicator for timing of pulmonary valve replacement.

Objective:
To evaluate the association between novel pre- and post-operative biomarker levels and 30-day unplanned readmission or mortality after paediatric congenital heart surgery.
Methods:Children aged 18 years or younger undergoing congenital heart surgery (n = 162) at Johns Hopkins Hospital from 2010 to 2014 were enrolled in the prospective cohort. Collected novel pre- and post-operative biomarkers include soluble suppression of tumorgenicity 2, galectin-3, N-terminal prohormone of brain natriuretic peptide, and glial fibrillary acidic protein. A model based on clinical variables from the Society of Thoracic Surgery database was developed and evaluated against two augmented models.
Results:Unplanned readmission or mortality within 30 days of cardiac surgery occurred among 21 (13%) children. The clinical model augmented with pre-operative biomarkers demonstrated a statistically significant improvement over the clinical model alone with a receiver-operating characteristics curve of 0.754 (95% confidence interval: 0.65–0.86) compared to 0.617 (95% confidence interval: 0.47–0.76; p-value: 0.012). The clinical model augmented with pre- and post-operative biomarkers demonstrated a significant improvement over the clinical model alone, with a receiver-operating characteristics curve of 0.802 (95% confidence interval: 0.72–0.89; p-value: 0.003).
Conclusions:Novel biomarkers add significant predictive value when assessing the likelihood of unplanned readmission or mortality after paediatric congenital heart surgery. Further exploration of the utility of these novel biomarkers during the pre- or post-operative period to identify early risk of mortality or readmission will aid in determining the clinical utility and application of these biomarkers into routine risk assessment.
Randomized controlled trial of a self-administered five-day antiseptic bundle versus usual disinfectant soap showers for preoperative eradication of Staphylococcus aureus colonization

Objective
To determine the efficacy in eradicating Staphylococcus aureus (SA) carriage of a 5-day preoperative decolonization bundle compared to 2 disinfectant soap showers, with both regimens self-administered at home.
DesignOpen label, single-center, randomized clinical trial.
SettingAmbulatory orthopedic, urologic, neurologic, colorectal, cardiovascular, and general surgery clinics at a tertiary-care referral center in the United States.
ParticipantsPatients at the University of Minnesota Medical Center planning to have elective surgery and not on antibiotics.
MethodsConsenting participants were screened for SA colonization using nasal, throat, axillary, and perianal swab cultures. Carriers of SA were randomized, stratified by methicillin resistance status, to a decolonization bundle group (5 days of nasal mupirocin, chlorhexidine gluconate [CHG] bathing, and CHG mouthwash) or control group (2 preoperative showers with antiseptic soap). Colonization status was reassessed preoperatively. The primary endpoint was absence of SA at all 4 screened body sites.
ResultsOf 427 participants screened between August 31, 2011, and August 9, 2016, 127 participants (29.7%) were SA carriers. Of these, 121 were randomized and 110 were eligible for efficacy analysis (57 decolonization bundle group, 53 control group). Overall, 90% of evaluable participants had methicillin-susceptible SA strains. Eradication of SA at all body sites was achieved for 41 of 57 participants (71.9%) in the decolonization bundle group and for 13 of 53 participants (24.5%) in the control group, a difference of 47.4% (95% confidence interval [CI], 29.1%–65.7%; P<.0001).
ConclusionAn outpatient preoperative antiseptic decolonization bundle aimed at 4 body sites was significantly more effective in eradicating SA than the usual disinfectant showers (ie, the control).

We quantified the time burden of alcohol-based handrub accompanying nonsterile-glove use among emergency physicians, through observation in controlled and clinical settings. We report gloving episodes per hour, gloving times with and without handrub, and handrub recommendations compliance. Handrub adds 46 seconds to each glove-use episode, and we provide national extrapolations.

A survey to determine the prevalence of diabetes in people with Down's syndrome was carried out at t raining centres in the West Riding of Yorkshire. Out of a total of 914 people known to the County Health Department as having this condition, 456 attended either junior or adult training centres and, of these, 404 provided urine specimens on two separate occasions. There were seven cases of diabetes among this group—a prevalence of 17 per thousand. Of more significance is the finding that the rate among children aged 0–14 years was 20·6 per thousand, which is considerably higher than expected in a population of this age group. It is postulated that there is an underlying defect of carbohydrate metabolism in people with Down's syndrome.
DNA-Directed Assembly of Anisotropic Nanoparticles on Lithographically Defined Surfaces and in Solution

Anisotropic, noble metal nanoparticles have been synthesized using a template synthesis strategy. In short, metallic salts are reduced in the nanometer scale pores of either an alumina or polycarbonate membrane. The particles can then been released from the template to form suspensions of anisotropic nanoparticles. These nanoparticles have been modified with deoxyribonucleic acid (DNA) oligomers of varying length using several different attachment chemistries. The thermodynamics and kinetics of modifying these particles with DNA has been explored. DNA has also been used to assemble the particles on planar Au surfaces as well as lithographically defined Au pads on Si wafers. In addition to surface assembly, DNA has been used to assemble the nanowires into simple, yet deterministic structures in solution.

Nanostructures are electrochemically deposited into alumina or polycarbonate templates resulting in monodisperse, anisotropic particles with a range of tunable sizes. These particles have been synthesized with diameters of 20–250 nm and with lengths of 1–10 μm. Currently, structures have been made with stripes of Au, Ag, CdSe, Co, Cu, Ni, Pd, and Pt. These materials offer a variety of different properties. In particular, many of the metals in this group are excellent conductors, meaning these particles can actually be used as nanowires. Co and Ni are ferromagnetic and may be used for separation or assembly. CdSe is a semiconductor, possibly allowing for the synthesis of electronic devices such as transistors. Furthermore, many of these materials have different surface chemistries, making the orthogonal functionalization and assembly of these nanowires more accessible. This research focuses on increasing the number of materials available, especially semiconductors, incorporating these potentially useful materials into multilayered nanowires and evaluating their electrical properties, either individually or in small bundles. In addition, the surface chemistry of the various materials in the nanowires is being compared to aid in orthogonal self-assembly of functional nanostructures such as memory devices. The work presented will demonstrate the effects of rod composition on electrical properties. In particular, the effects of changing the work function of the materials on either side of a semiconductor to form Schottky junctions or ohmic contacts will be shown.
